Common Questions

How much does a SAP evaluation cost?

The $95 Pre-Assessment Questionnaire is provided to your SAP and gives them the needed information to begin their portion of the Return-to-Duty process with you.

The SAP’s fee (up to $500) is paid directly to the SAP and generally includes the initial evaluation, follow-up evaluation, and Return-to-Duty recommendation review.

Every situation is different. Depending on the SAP’s recommendations, additional education, treatment, or follow-up services may be required. Any fees associated with those services are separate and are determined directly between you and your SAP.

Total Return-to-Duty process cost as listed above: up to $595.

How does the SAP Evaluation process work?

Step 1: Select your state and pick your SAP, then purchase the $95 Pre-Assessment Questionnaire. Step 2: Complete the questionnaire, schedule and attend your Virtual SAP Evaluation, then complete your SAP’s recommendations. Step 3: Attend your Virtual Follow-Up Evaluation.
