- Client Users: When you complete the Pre-Assessment Questionnaire, we collect your name, email address, and phone number.
- SAP Providers: When you apply to be listed in our directory, we collect your name, email address, phone number, professional address, credentials, qualifications, training certifications, professional biography, headshot photograph, states served, languages spoken, and credential documentation.
Information Collected Automatically:
- Usage Data: We may collect information about how you access and use the Site, including your IP address, browser type, operating system, referring URLs, pages viewed, and dates/times of visits.

- Operate and maintain the FindaSAP directory and referral platform
- Connect clients with qualified Substance Abuse Professionals

- SAP Providers: When a client selects a SAP through our platform, we share the client’s relevant contact information with that provider to facilitate the evaluation process.
- Service Providers: We use third-party services to support our operations, including:
- National Online Training / EZLCMS — a learning management platform licensed and operated by Court Solutions LLC, used to process the Pre-Assessment Questionnaire and collect the $95.00 fee
- Box Sign (Box, Inc.) — to send and execute Provider Agreements electronically
- Convesio (Convesio, Inc.) — to host our website
- Legal Requirements: We may disclose your information if required by law, regulation, legal process, or governmental request, or to protect the rights, property, or safety of FindaSAP, our users, or others.
4. Data Retention
We retain personal information for as long as necessary to fulfill the purposes for which it was collected, including to satisfy legal, accounting, or reporting requirements. Specifically:
- Client data: Retained for the duration of the client’s Return-to-Duty process and for a period of three (3) years thereafter, unless a longer retention period is required by law.
- SAP Provider data: Retained for the duration of the provider’s listing in our directory and for one (1) year following removal.
- Provider Agreements: Retained for seven (7) years following execution, consistent with standard business record-keeping practices.
